What to Know Before Building a Dental Office

A guide to the decisions you'll face when planning a dental office buildout.

Dentists who are building their first office, and dental groups opening additional locations, face a construction process that's more specific than most commercial buildouts. The equipment decisions, the utility infrastructure, and the coordination required between the contractor and the dental equipment supplier all happen before framing starts. Getting those decisions wrong early creates expensive problems late.

Start With the Practice Workflow

The first thing that determines how a dental office is built is how the practice is going to work. How many operatories? Where is the sterilization area relative to the operatory flow? Where does the front desk sit relative to the patient entry? Where does the X-ray or imaging room need to be?

These questions sound like design questions, but they're really construction questions. The answers determine where the dental utility connections go, which determines where the walls go, which determines what the structural and plumbing requirements are. If you don't answer these questions before the design is finalized, you'll answer them in the field, and that's when changes are expensive.

Plan Around Equipment and Utility Needs

Dental equipment drives the utility layout. Each operatory requires dental air (compressed air), vacuum, water supply, drain, and power connections specific to the chair and delivery unit being installed. The sterilization area requires specific plumbing configurations for autoclaves and ultrasonic cleaners. Imaging rooms require electrical for the X-ray unit and, in some cases, lead lining in the walls.

All of this has to be coordinated between the dental equipment supplier, the architect, and the contractor before the construction documents are finalized. The equipment supplier should provide rough-in drawings that show exactly where each connection needs to be. The contractor needs those drawings before framing starts so the utility connections are in the right locations before drywall goes up.

Understand Budget and Schedule Drivers

Dental office construction costs more per square foot than a standard commercial office buildout. The utility infrastructure alone, including the dental air compressor, vacuum system, plumbing to each operatory, and specialized electrical, adds significant cost that doesn't appear in a generic commercial construction estimate.

The other major cost driver is the existing condition of the space. A vanilla shell space in a new commercial building is a clean slate. A second-generation space in a former retail or office use may have utilities in the wrong locations, requiring demolition and rerouting before new construction can begin. Getting a contractor walkthrough of any space you're seriously considering before signing the lease is the most effective way to understand what the buildout will actually cost in that specific space.

Schedule-wise, dental buildouts are typically faster than medical construction but slower than standard office tenant improvements. Allow for permit review time, which varies by jurisdiction, and for dental equipment lead times, which can extend to 8-16 weeks or more for some equipment categories.

Coordinate Early With the Right Partners

A dental office buildout typically involves at least four parties outside the contractor: the dental equipment supplier, the dental equipment planner or designer, the landlord's engineer (who has to approve utility connections to the base building), and the local building department. These parties have to exchange information in a specific sequence, and the sequence has to be coordinated before anyone starts drawing or building.

The contractor is the best person to manage this coordination because they're the one who has to build to the result. Involving the contractor early, before design is finalized, is the most reliable way to make sure the coordination happens in the right sequence.

Before signing the lease, if at all possible. A contractor walkthrough of the space you're considering, before the lease is executed, provides accurate cost information for the buildout in that specific space. That information is useful for lease negotiation, for understanding the real gap between the landlord's improvement allowance and the buildout cost, and for planning the construction timeline relative to your target opening date.

The most common mistake is finalizing the lease and starting design without contractor input on what the space will actually cost to build. This creates a situation where the design is completed, the contractor is hired, and the first real budget is significantly over what the dentist assumed. At that point, changes are expensive, the lease start date is set, and the options are reduced. Early contractor involvement prevents this.

You don't need a contractor who only does dental work, but you need a contractor who has built dental offices before and understands the coordination requirements. A contractor who has never dealt with dental equipment rough-in, treatment room utility layouts, or the equipment supplier coordination process will learn on your project, and that learning is expensive. A contractor with dental experience already knows the process and manages the coordination without a learning curve.
